Issue with form formatting

I have purchased and installed the Premium version, and the issue with the error is resolved and the redirect to the thank you page works perfectly.
However, the formatting for the form that is displayed is a bit wonky. You can see the form here:

I am not using the PayPal or Authorize.net functionality, and am currently not using MailChimp (but will probably do so). Can this display be worked on somewhere? Is there a CSS file that can be fixed?

Hey Chuck,
Thanks for your purchase. The above link returns a 404 error. Anyway you can attach a screen of what the form looked like? If you would like to make adjustments to the CSS you can by editing front.css in the plugin folder.

We have it running on 2 test servers and cannot replicate the issue. If you can/want would you be able to email me temporary ftp and wp-admin info so we could troubleshoot specifically on your installation to see what is causing the problem? I am using the Premium version right now at:

I have configured the shortcode as follows:
[register_webinar webid= 7097007067864815106 type=single]
If you go to the page above, you can see that the formatting for the table is off.
As far as having you test on the server, it might be difficult to give you access based on our security policies.

For the formatting of the form in Premium that you link to above did you modify styles in front.css in plugin folder to accomplish the look you desire?

There is no front.css file in the folder for GoToWP Premium. I have admin.css and public.css in the root. No front.css.
Should it be there?
